Up all night, or how I learned to deal with Insomnia.

Let me first start with a story.

Awhile ago I decided I was finally ready to have children. And then when most people were planning for their retirement; I had twins. The one criteria the hospital needed to release me, after the birth of my twins, was making sure the children could tolerate time in a car seat. They passed the car seat test and I was discharged. I knew nothing, though, about how to raise a child, much less twins. Not how to change a diaper, not how to feed a child. Nothing.

In my last trimester of pregnancy I had some insomnia and of course after the twins were born sleep, especially for longer than three hours, was hard to come by. After about a year and a half I was exhausted but also elated. The twins were finally sleeping through the night and maybe now I would finally be able to get some serious shut eye, but surprisingly I could not.

I would lay in bed all night exhausted but sleep would not come. I would get out of bed. I would do boring things and than get back into bed. I finally started playing “musical beds” and would try sleeping in various beds throughout the house. This worked, but only for a short time. My insomnia finally evolved into a very sad pattern. I would be up for two days and two nights, doze for around four hours and than be up again for two nights. This painful pattern persisted for two years. Meanwhile, I had two young children to raise. In retrospect I see those years as my “dog paddling years.” I was just trying to survive and keep my head above water.

Hang in there, it will get better.

After some of the extreme stress in my life decreased suddenly I was able to sleep. I’m still not a great sleeper but I’m better. And in the meantime I learned, the hard way, about how to manage my insomnia.

When you can’t sleep you want to begin your journey with a trip to your doctors office. As a general practice it is a good idea to consult with your healthcare provider first about any medical conditions you may be experiencing and whether these are contributing to your insomnia. You also might want to include your pharmacist in this conversation as sometimes people are unaware that a medication they are taking contributes to or even causes insomnia. Your clinician may recommend a sleep study which can determine if there are physical aspects to your sleep disorder, if so you may be referred to a doctor who specializes in sleep disorders. Most medical doctors who treat “sleep disorders” do not treat insomnia.

A lot of times sleep disorders occur alongside mental health issues; so it can be helpful to see a psychotherapist or a psychiatrist who may prescribe psychiatric medications to treat a depression or anxiety disorder. For example, after major surgery many people experience a depression and insomnia can be a symptom of depression or an individual may be experiencing the manic phase of a bipolar disorder and may not feel sleepy, and therefore may not want to go to bed; usually a psychiatrist will treat these disorders with psychotropic medications in conjunction with a psychotherapist providing counseling services.

The different types of Insomnia

There are generally three different types of insomnia: inability to initiate sleep, inability to maintain sleep and/or the tendency to wake up before your alarm. People may also cycle through all three types.

The first category is usually called delayed sleep phase disorder and is considered a little bit different than the others (this category might be similar to an individual who is a “night owl.”)

This type of insomnia may be caused by a circadian rhythm disorder and may benefit from light therapy; in addition to the recommendations listed below.

In terms of the other two categories of sleep disorders, there are a host of ideas about what one can do to improve their sleep.

Some Tips

For episodic insomnia usually people are encouraged to follow good sleep hygiene. I would also recommend avoiding sleeping pills, except for short term use.

Some good sleep hygiene recommendations are:

-Creating a bedtime routine consisting of a wind down period. Try to go to bed and wake up at the same time, even on weekends.

-Make sure your bedroom is comfortable (eg, dark enough, quiet enough, the right temperature) for a good night’s sleep.

-Have a white noise machine in your bedroom or a machine that mimics relaxing sounds. A constant sound like the hum of a fan can really help to drown out both extraneous sound and any internal chatter that may be keeping you awake at night.

-Don’t exercise before bedtime, also try to stop eating about three hours before bedtime.

-Explore taking a hot bath about an hour and a half before bedtime. Changes in body temperature can help to trigger sleep.

-Use your bed only for sleep and sex. The room(s) we live in tend to become associated with the events that occur within them. So if you are on a diet and don’t want to snack between meals, get out of that kitchen. If you want to sleep better try to only associate your bed with restful sleep and sex. These associations happen quickly, tossing and turning in bed for even an hour can create this negative association so if you can’t sleep for twenty minutes or so get out of bed and engage in a relaxing or routine activity. When you become sleepy get back in bed and try again to fall asleep.

-Do not expose yourself to bright light before bedtime, this stimulates your brain into thinking it’s morning time. Most tablets and phones have a blue light option which can help to filter out the light that stimulates your brain.

-Turn your bedroom alarm clock around so it’s not facing you.

-Limit products with caffeine in them after lunch time. And remember most “brown” sodas have caffeine in them.

Additional Suggestions

Some people have found taking a therapeutic dosage of melatonin helpful for initiating sleep. Melatonin is an over the counter product which is a hormone that is secreted by the body naturally when you are sleepy.

I also recommend journaling to my clients. Write down the events of the day and how you felt on that day. On the nights that you can’t sleep are you noticing some themes in your journal? Ideally, it’s better to partner with a psychotherapist that you’ve developed a trusting relationship with to help you figure out how any of these themes might be interfering with your sleep.

Take a yoga class. Anxiety can be felt physiologically and yoga can help to stretch out your body and relieve some of the physical symptoms of stress.

The techniques of Progressive Relaxation and Meditation

If you find that it’s 4:00 am in the morning and you are still staring at that alarm clock try to:

Turn that alarm clock around.

Get out of bed.

And try progressive relaxation, deep breathing and meditation exercises. Progressive relaxation is a generic term for using guided imagery to relax yourself. For example, you might listen to a recording of a comforting, relaxing voice listing each part of your body; feeling the tension, for example, in your hands and fingers by clenching your hands into a fist and than relaxing them and noticing the difference in these two feelings. The guided imagery would than list all the parts of your body that usually experience tension, emphasizing relaxing each separate part of your body. And finally as your whole body feels more relaxed it can be easier to slip into sleep. People sometimes incorporate these recordings into their sleep routines.

Practicing meditation can also be helpful in managing your daily level of stress. Here’s how I think of meditation. When you’re anxious your mind can go skittering off in many directions. One tenant of meditation is mindfulness, being present in the here and now. Mindfulness discourages skittering off into the past and future. And meditation teaches you various techniques to lasso those thoughts and herd them together down the particular path you want them to follow. Deep breathing techniques help to make sure your body is oxygenated, which is naturally relaxing.

So now armed with your new techniques get back in bed and practice them. Take a deep breath in and watch as your diaphragm expands (place your hand on your stomach, if your hand raises your diaphragm is expanding,) slowly breathe out of your nose while simultaneously counting up from one slowly. If you find yourself doing anything besides focusing on your breathing and your counting, bring your mind back to taking deep breaths in, breathing out through your nose, and start counting again. Develop your own relaxation techniques and teach that pesky mind of yours that when you do your deep breathing exercises the only activity that you want to follow is a long, restful nights sleep.

CBT-I and Sleep Restriction

A current trend in the treatment of insomnia is to see a technician or ideally a clinician that incorporates the techniques of CBT-I into their treatment regime. CBT-I stands for Cognitive Behavior Therapy and the “I” stands for Insomnia. CBT therapy may or may not include medication in the treatment, depending on whether the treating clinician views this as indicated. CBT encourages the client to examine how their thoughts affect their mood and tends to include assignments and homework. Also, these clinicians may incorporate sleep restriction interventions along with assignments into their treatment for insomnia.

Insomnia can become something we manage for life but as I’ve become better acquainted with my particular type of insomnia I’ve developed insight into my relationship with sleep and I have become better at identifying what triggers my insomnia and have developed strategies for managing my sleep. All of these techniques have not lead to a happily ever after finale to my story but they have certainly helped in my finding a better than usual ending to my sleep disorder.

Breaking news: the United States Department of Justice has just received a “cease and desist” letter from Margaret Atwood’s lawyers.

You may remember Ms. Atwood as the author of the book the Handmaid’s Tale which describes a dystopian future land named the Republic of Gilead. This land consists of a “totalitarian, economic and neo-Puritanical regime that in the future takes over most of the continental United States of America.” Ms. Atwood’s lawyers contend that with this most recent election, the United States has moved closer to duplicating Ms. Atwood’s fictional Republic of Gilead and has therefore infringed upon the copyrighted material as described in Ms. Atwood’s book.

Mr. Diego Gonzalez of New York City, was captured yesterday after hiding on a Delta Airlines flight from New York City to Paris, France.

Upon arrival at Charles de Gaulle International airport in Paris, Mr. Gonzalez was swiftly handed a croissant. While passing through customs and immigration Mr. Gonzalez did not have a passport and when security arrested him he exclaimed “I was planning on going back. Honestly. My wife, Colette, is very sick and I’m going to visit her.” Officers then telephoned the number Mr. Gonzalez had given them as his contact number in Paris. According to reports the call was forwarded to the following message: Merci d’avoir appelé Dominoes. C’est Lucas. Comment puis-je vous aider aujourd’hui? (Nutle Reader’s official translation: Thank you for calling Dominoes. This is Lucas. How may I help you today?)

In support of the Republican catchphrase “Make America Great Again” Elon Musk has announced a one-million-dollar prize to the winner of a duel to the death at the Caring Hearts nursing home facility in Frackin, Pennsylvania.

Ida Schmutz, 85, of Kalamazoo, Michigan subsequently challenged Elmer Schmidt, also 85, of Frackin to a duel to the death. Mr. Schmidt gleefully accepted the challenge while stating “People today are living way too darn long, anyways.” The winner will be awarded one million dollars from the Elon Musk, Make America Great Again By Buying It All Up, fund.

Whoever is first to knock the challenger off their walker (or wheelchair) and either rollover or stomp their opponent to death will pocket the purse. Mr. Derrick Rumrunner, a janitor at the facility, will be monitoring the duel and will render medical treatment, in the event of a tie.

The duel will be carried over Truth Social, the official social media platform of New America, and was set to begin this Thursday afternoon at 3:00 pm (EST) outside the Caring Hearts nursing home, but residents there worried about missing dinner. The time has subsequently been changed to 5:00 pm, (MST) but administration has warned that at this time residents are at risk for “Sundowner Syndrome.” The Trump delegation offered that this would be an ideal time for a duel to the death as the residents would naturally be irritable AND confused.

Newsflash, this just in: Parent builds a moat filled with water and crocodiles surrounding her house, then adds a catapult, to deter college graduate from returning home. Mother quoted as saying: “my basement floods anyway, so he couldn’t live down there.”

The Nutle Reader asked generation Z to comment on this news story, with one reader noting: “I would have dug that damn moat myself if I thought she could pay me.” Most readers when queried for a response felt that: “if she didn’t want her basement to flood, she shouldn’t have built the moat.”

You may remember Ms. Channing as president elect Donald Trump’s newly appointed ambassador to Russia.

As part of a Trump transition team visiting the Russian ROSZDRAVNADZOR bureau (the Federal Service on Surveillance and Social Development of the Russian Federation) headquarters in Moscow; Ms. Channing is said to have slipped on a banana peel while simultaneously ricocheting out of a 10 story window, falling to her death below.

In President Trump’s eulogy to Ms. Channing, his first ambassador pick, he is quoted as saying:

…Though there are good people on both sides we mourn the loss of a terrific and fantastic comedian, singer, dancer and a person of diversity. Newsflash: Darryl Jones, CEO of the IB Frackin corporation an LLC headquartered in Pittsburgh, Pennsylvania, announced today that one of his crew members recently unearthed the “Deep State” hidden beneath the mantle of the earth’s crust. “It wasn’t really even hidden that deep,” Mr. Jones noted.

The president-elect has called in the military to help with the removal of the deep state from the earth’s crust, though reportedly there has been a delay, as calls to the Pentagon remain unanswered. Unnamed sources tell the Nutle Reader that, “the battery on the phones at the Pentagon just died, though they are currently charging the phones.” Also, one other Pentagon worker’s mother was sick, so she was unable to get to the phone on time. Our source assured us that they are planning to return the calls “just as soon as their phones have completed charging.”

In the news: when asked to clean his room, a 13-year-old yelled at his mother: this is a “tyranny of the masses!” His mother made him clean his room, anyway.

Ireland and the U.S agree to trade foreign immigrants

After months of intense negotiations, representatives of President elect Donald Trump and his Excellency Michael Higgins of Ireland, have agreed on a trade deal. In order to address pressure from citizens groups over the issue of illegal immigrants flowing unchecked to the shores of both countries; the world leaders have decided that they will now take turns trading foreign nationals.

On Mondays, Wednesdays, and Fridays a boat will sail from Ireland to America filled with illegal immigrants, and on Tuesdays, Thursdays and Saturdays the boat will return from the U.S. back to Ireland, carrying its cargo of foreign nationals’ home. The boat will not sail on Sundays. The world leaders hailed the announcement as “really great by almost everyone” over a dinner of soda bread and blood pudding, which a staffer kindly removed before anyone could start eating.

President Trump announced today that instead of a wall dividing the United States from Mexico he will now be recruiting angry “Incel” men to patrol America’s southernmost border. He also announced that he will be issuing AK-47’s and ammunition to his newly created brigade. Mr. Trump has declared “All of this will be completely free as the men will gladly pay to provide this terrific and fantastic service.”

In an end to an era of political correctness these armies will be named “the Macho Force.” As this was announced citizens in Texas, Arizona and Nevada could be heard exhaling a concerted sigh of relief while women in California executed a collective face plant into the palms of their hands, which scientists estimate registered as a 7.0 on the Richter scale at the California Loma-Prieta fault line.

News flash- this just in
Commander, the Biden’s German Shepherd mix, has been seen chasing Malania Trump through the Whitehouse Rose Garden, as the First Lady-elect is heard yelling “ta pes je grožnja!” (Nutle Reader’s official translation: “that dog is a menace!”)

Hunter Biden is said to have commented “I could have sworn I tied that damn dog up.”

This just in: Rap artist Chief Keef mugged at gunpoint while stopping at Starbucks in Vallejo, CA.

Chief Keef, a rapper out of Chicago, was on his way to Sacramento, CA to perform a show when his band pulled their van into a parking lot and stopped at a Starbucks in Vallejo, CA. While in the store a witness ran into the Starbucks saying “some guys are smashing into a van.” When the band came out they were confronted by a robber, who stole all their computer equipment, but left their instruments.

When the band’s manager telephoned the Vallejo police department about the robbery in progress she was told that she could come into police headquarters and file a report, while pedestrians passed by the thief seemingly unphased by the gun toting robber. Chief Keef was heard to mutter under his breath, “this is fu@&ed up.”

Let America Be America Again

Let it be the dream it used to be.
Let it be the pioneer on the plain
Seeking a home where he himself is free.

(America never was America to me.)

Let America be the dream the dreamers dreamed–

Where never kings connive nor tyrants scheme that any man be crushed by one above.

Is crowned with no false patriotic wreath,
But opportunity is real, and life is free,

Are We Doomed to Forever Repeat the Past?

Recently I’ve been thinking a lot about Stanley Milgram’s 1960’s research study. I don’t know why.

If you don’t remember the study this is my synopsis: After World War ll Milgram, a social psychologist, was wondering why a majority of the German people during world war ll seemed to willingly comply with a fascist dictators cruel orders. In other words, why did (do) seemingly reasonable, ethical people comply with unethical authority figures?

Which got Milgrim thinking: maybe it was because German children were socialized to be compliant. Maybe the parenting style in Germany was more autocratic; America could then be considered to be inoculated from fascism, as the parenting styles in America were so different, so much more egalitarian and democratic, fascism would never dare to rear its ugly head again, especially in America.

So he created a test to see if his hypothesis was correct: Would American’s comply with directions to hurt another human being, even when ordered to do so by a person of authority, or would they become “whistle blowers” railing against the injustice? He took bets from other psychiatrists and psychologists. Most said no, Americans would not do it. They would not knowingly hurt another human being, even if directed to do so by a person of authority.

What do you think the answer was?  Around sixty-five percent of the subjects complied with the authority figures directions to hurt another person. (No, they were not actually hurting someone else.) Which leaves about thirty five percent left. Thank goodness for those thirty five per centers; the whistle blowers willing to push against tyranny, or else we might just be doomed to forever repeat the past.

How to Cope with a Bully-The Foundation

 

What can you do if you find out that your child is being bullied?

Tell your child to ignore it, don’t engage, turn the other cheek, tell an adult? What if I were to tell you that all of those suggestions might not help and when implemented some might even escalate the bullying behavior.

The world of children mirrors the world of adults so that we can gain clues about bullying from this world, the adult world that surrounds every child.

What would you do if you felt that you were being bullied, perhaps at work by a colleague or boss?

How would you handle that? What would you do? Ignore it, don’t engage, turn the other cheek, tell somebody, perhaps human resources? What if I were to tell you that all of those suggestions won’t help and when done some might even escalate the bullying behavior.

Bullying is a complicated phenomenon; with the trickiest part, the most complicated part of bullying, being the reciprocity that the bully shares with their environment. Bullying is interwoven into how organizations work, what its expectations are. All of these factors serve to reinforce bullying. These dynamics can even serve to keep the tormentor invisible to onlookers; unless you choose to look, bullying may seem to be completely normal. But it is not.

What does bullying look like? What are its symptoms, a bloodied nose? A broken arm? Sadness? Depression? A suicide attempt? People in general would disagree about what constitutes bullying, which in itself is interesting, but one definition of workplace bullying is when a bully targets a power subordinate with “repeated, health-harming mistreatment of one or more persons (the targets) by one or more perpetrators. It is abusive conduct that is: threatening, humiliating, or intimidating, or work interference — sabotage — which prevents work from getting done, or verbal abuse.

Workplace bullying is driven by the perpetrators need to control the targeted individual(s). It is initiated by bullies who choose their targets, timing, location, and methods. It is a set of acts of commission or omission (by withholding resources from others.)

It requires consequences for the targeted individual. It escalates to involve others who side with the bully, either by commission or omission. It undermines legitimate business interests when bullies personal agendas take precedence over work itself. And it is akin to domestic violence at work, only the abuser is on the payroll” 1

The difference between playground bullying and adult bullying is that adults are playing for bigger stakes: employees have families who depend on their livelihood in order to survive.

The backbone of bullying is the power hierarchy. Power hierarchies are integral to how a bully works, as bullies know how to charge up the ranks of any hierarchy, they then understand the opposite: not only must you be deferential to those above you, but you may also kick at those below you, without any fear of repercussions.

Anybody who pushes back against a bully is also pushing against this organizational body, this spine of power that the bully counts on; the up/down power hierarchy. Pushing against this body of force is very difficult for any power poor individual. This is why gangs exist, there is protection and power in numbers.

If you are being bullied at work, you are not alone. When there is abuse within a home all the family members may not acknowledge it, but that doesn’t mean they are not aware that the abuse is occurring. Your colleagues might even be experiencing a type of survivors’ guilt, torn between a fear of speaking up, thereby running the risk of being targeted or staying silent, and sticking their head in the sand, in an effort to keep their job. All of these ingredients combined create a toxic soup called the dysfunctional work environment. And of course, all of this toxicity draws energy away from the tasks at hand and into the morass that becomes another day on the job.

Most people who speak about bullying talk about the target and not the army of people that surround and protect the bully. You don’t have to go very far to see this dynamic in action. I bet you could even pick up a copy of today’s newspaper and see dozens of instances of bullying, enabling and multitudes of people frozen in place wondering how to handle and manage the bully. So most certainly you are not alone.

It’s considered socially taboo in our culture to openly discuss power differentials, though society is predicated on these differences. A bully counts on this. Bullies are cowards. Fear is their achilles heel.  They will assess a power hierarchy before picking a target, because they need the rules of the game to back them up. And they will use these differences to add wallop to their punch.

So going against a bully, alone, is difficult and in some instances it might even escalate the attacks. It can get better though, mostly because you can make it better. In future articles I will discuss some techniques for managing the bully at work.

Parenting, a cautionary tale:

What causes poverty?  Is it growing up poor? How about growing up as part of an ethnic or power minority? Perhaps lack of access to an education? I am currently, simultaneously, typing on my computer while watching TV commercials: there’s a happy mother vacuuming as her children, husband and dog leave a trail of crumbs behind on the carpet. There’s a fussy mom choosing peanut butter and another dad taking pictures of a happy mom and child and then transferring his pictures to his computer.  Most households, I must than conclude, consist of bright happy families, a dad, mom and two children, who gratefully consume products and services.  Except that two parent families are in decline and are now a minority. Most households, fifty seven percent, consist of only one or two adults. Households with children, although a minority at forty-three percent, do consist of a growing segment of the population: poor mothers and their children.

Once upon a time my dreams came true. I was married. I still am. The day of my wedding was magical. White dress, bridesmaids, my husband by my side, friends and family in attendance. I wanted my wedding to formalize this new stage in my life, to introduce a new “us” to family and friends. Suddenly, though, a formerly silent world opened up in front of me.  A bed room scattered with Ken and Barbie dolls, easy bake ovens, and dolls that you fed, changed and pulled on a string, on their back, only to hear them repeat the word “mama” over and over again.

I remember playing mommy and daddy with my best friend Debbie: I would serve her a hearty breakfast made in the miniature replica of a kitchen that we both had, toast it up nice and (very) brown in my ever present oven and serve it up to her as she would prepare to walk out the door, on her way to work. “Have a nice day” I would intone. I would then industriously begin to prepare dinner in anticipation of my ‘husband’s” arrival home from work.  I must admit though that soon into breakfast this game would end as Debbie and I would veer our attention off into the next game. We also practiced being brides, choosing our bridal dresses, shoes and menus for that all important day. And now, as I looked out on the filled banquet hall I could feel myself suddenly shrinking back into that 10 year old little girl, standing in her mother’s white heels and dress. Suddenly, long silent childhood hopes and dreams began to take over.

I wanted to be a beautiful bride, carried away in a horse drawn carriage to live happily ever after. The day had to be perfect. As it got closer to the wedding date I began to feel like a “wedding mole.” Somebody who, when pressed into service, had become someone else. A cold war bridal operative who suddenly expected, on her wedding day, that the pumpkins would turn into a carriage and the mice would suddenly morph into horses. What would happen next? Would I break out into song, perhaps something about finding my prince? At least I had won the argument with my mom; the wedding reception would have an open bar.

And they all lived happily after. Here’s where I think fantasy and reality may drastically part ways. The marriage fantasy can change dramatically after children. This road may not only be a rite of passage for women; it can lead to an uncertain future for a woman and her children. As the life expectancy of most marriages hovers at around a 50% and women, after a divorce, tend to retain physical custody of the couple’s children some statistics begin to emerge:

Women are paid around 22% less than men for the same work. Women tend to find their way to lower paying fields such as jobs in social work and education. 1  Out of the minority of forty-three percent of households that still have children present in the home, a majority of these are single parent, female headed households. Seventy three percent of these households live at or below the poverty level 2; so that a majority of children who live in female headed households are growing up, living in poverty.

Perhaps happily ever after is a fantasy, but for a woman and her children, it may not only be the end of an illusion, it may also be the beginning of her downfall.

Then they came for the African Americans, and I did not speak out—

Because I was not African American.

Then they came for the Asians, and I did not speak out – – –

Because I was not Asian.

Then they came for the Hispanics, and I did not speak out—

Because I was not Hispanic.

Then they came for the Foreigners, and I did not speak out—

Because I was not a Foreigner.

Then they came for the poor, and I did not speak out—

Because I was not poor.

Then they came for the middle class, and I did not speak out—

Because I was not middle class.

Then they came for the elderly, and I did not speak out—

Because I was not elderly.

Then they came for the Intellectuals, and I did not speak out—

Because I was not an intellectual.

Then they came for Women, and I did not speak out—

Because I was not a woman.

Then they came for me—and there was no one left to speak for me.
